| Corporate Membership 10% Discount (20 employee units minimum)
15% Discount (35 employee units minimum)
The discount only applies to full year memberships. A business
representative will collect a non-refundable $25 deposit for each single membership or $50
deposit on couple/SPF/Family memberships. This will be submitted with the list of
employees to the Life Fitness Center staff, so the transaction can be completed. The
deposit will be deducted from the balance due. This is a commitment for the membership and
for the deduction. A balance due sheet will be forwarded to the business representative.
When the individual comes in to fill out paperwork and start membership, they are
responsible for the balance due. If the individual does not sign up for membership, the
group is not penalized, but the individual does not receive the deposit back. Memberships
must be renewed each year as part of a corporate plan to continue to receive the discount.
Payment is due in full at time of activation. Refund policy is the published policy listed
in the Fitness Center brochure. Please call to set up a time to meet with a full-time
staff member to discuss our corporate program. |

checking account as a payment option. This plan requires a set down payment (this amount
varies with membership types), voided check, and a current checking account under member's
